Application for Registration of a Candidate for a Local Government Election (EF.670)  (61 kb)
Local government candidates must be registered for a local government election before accepting political donations for the election. Any person wishing to accept political donations for a local government by-election or the 2012 local government general election must complete and lodge this form.

Local government groups must be registered for a local government election before accepting political donations for the election. Any group of candidates wishing to accept political donations for the 2012 local government general election must complete and lodge this form.

Notice of Appointment of Official Agent (EF.672)  (61 kb)
Complete this form to appoint an official agent. Local government candidates and groups must appoint an official agent before receiving $1,000 or more in political donations or incurring $1,000 or more in electoral expenditure. Appointment of official agents for the 2008 local government elections is now closed. Appointment of official agents for local government by-elections close on the day before polling day for the by-election.

Disclosure of Political Donations and Electoral Expenditure for a Candidate (EF.678)  (74 kb)
Each person who was a local government candidate at any time between 1 January 2009 and 30 June 2009 is required to complete this disclosure form. If the candidate has a registered official agent the agent is responsible for completing and lodging the form. The disclosure form is to be lodged with the EFA between 1 July and 25 August 2009.

Disclosure of Political Donations and Electoral Expenditure for a Group of Candidates  (63 kb)
The head candidate of each local government group that contested an election at any time between 1 January 2009 and 30 June 2009 is required to complete this disclosure form on behalf of the group. If the group has a registered official agent the agent is responsible for completing and lodging the form. The disclosure form is to be lodged with the EFA between 1 July and 25 August 2009.
